Registration

New Student Registration Requirements:
You must have two of the following to verify residency: 

a.) Copy of closing documents on the home, or copy of the sales contract. *

b.) Receipt of rent paid (if renting)

c.) Rental lease agreement

d.) Driver's license with current district address

e.) Utility bill with name & address

f.) Notarized letter from homeowner stating that relative will be residing at this address and two forms of identification with address from homeowner

Additional Student Information:

a.) Original birth certificate with seal affixed to it

b.) Current physical/immunizations (Form is available on this page.)

c.) Custodial papers if that applies

d.) Completed registration forms. Forms are available at the district office, 28855 N. Fremont Center Rd., Mundelein.

Kindergarten Registration Requirements:
a.) Original birth certificate with seal to verify that the child is 5-years-old on or before September 1.

b.) Proof of current physical/immunizations (Form is available on this page.)

c.) Current dental examination (Form is available on this page.)

d.) Current eye examination (Form is available on this page.)

e.) Completed registration forms. Forms are available at the district office, 28855 N. Fremont Center Rd., Mundelein.

Second Grade Registration Requirements:
a.) Dental examination

Sixth Grade Registration Requirements:
a.) Physical examination

b.) Dental examination

These documents are needed at the time of registration. If these are not received at the time of registration, it will not be completed until these are received.

*If house closing takes place after school begins, a copy of the sales contract is needed. Also, parents are required to pay an out-of-district tuition fee for each child in the district. The check will be returned if the closing is within 90 days of when the child begins school at Fremont. If the closing does not occur within 90 days, parents will be responsible for tuition until the closing dates, as per Board policy.
