Health Forms
It’s not too soon to begin scheduling doctor visits for the next school year. Please drop off forms to your child’s school nurse or fax them to 847-566-7280 before the first day of school. For copies of forms or immunization information, please visit the links on the Health Services website page.
Incoming Kindergartners must have on file:

Nurse News
Health Form Deadline – October 15: Required health/immunization records must be submitted to the school nurse prior to October 15 of each year. Students who are not in compliance are excluded from school, per the Illinois School Code until records are complete and submitted. Medical forms can be submitted during open registration days, or to the district office over the summer. Illness:
Parents should keep a child home when these symptoms are present:
● Fever (within the last 24 hours of 100 degrees or more)
● Nausea or vomiting
● Skin rash or sores
● Reddened, swollen, or watery eyes
● Red or swollen joints
● Diarrhea
● Excessive coughing or sneezing
● Sore throat
● Headache or earache
Children should be kept home for 24 hours after an elevated temperature has returned to normal without fever-reducing medication. Parents are asked to keep their child home for 24 hours after vomiting or diarrhea. If any of these symptoms persist, please contact a qualified physician. Any students not attending school during the school day may not attend special school events (including holiday parties) or after school activities.
Parents will be informed by email or letter if/when exposure to a contagious illness is known. Parents can help curtail the possibilities of epidemics by daily inspection of their own children. Because of the contagious nature of many illnesses, please consider the child’s physical complaints before leaving or sending a child to school. If a child displays any symptoms of illness, parents will be contacted and expected to arrange for the student to be picked up. Sick children cannot remain in school.
If a child is on an antibiotic, he/she may not return to school unless he/she has been on medication for a 24-hour period as well as fever free for 24 hours.
Mission
Children’s health is a key element in positive educational outcomes. The goal of Fremont School District 79 Health Services and their school nurses is to promote, protect, support, improve, and sustain the health, emotional well-being, and safety of all students and staff for optimal health, growth, learning, and quality of life. This is accomplished in collaboration with the students, families, staff, administrators, and community resources.
Parents are asked to provide important health and medical information about their student, which is updated each year during the online registration process. Please complete this information as accurately as possible, as this information will accompany students to the hospital if emergency treatment is needed. It also enables the school nurse to understand ar child’s health needs at school. If there are changes during the school year, please be sure to notify the nurse. This health information is maintained by the nurse in each building and is kept confidential. Please feel free to contact the school nurse regarding any concerns about health or medical needs.

Confidentiality
Discussions between the nurse, students, family members, or staff are private. All participants are welcome to talk openly about themselves, their health, or questions that they may have. The information is not shared with other people without permission. The only exception is if someone may be in danger. If a person prefers a more private place to talk, those arrangements will be made. Student medical records are confidential, as well. They are not released without parental consent. Written parental authorization is required for the school to obtain medical records from outside sources.
Attendance
Daily school attendance is required by the School Code of Illinois. Student absences for any reason other than illness are strongly discouraged. If your child is going to be absent from school, parents must call the school by 9 a.m. on each day of absence to report and explain the reason for the absence. Please call the nurse's extension listed above if your child is going to be absent from school.
Illnesses at School
If a student becomes ill during the day, the school nurse will check the child and, if necessary, call a parent or emergency contact to pick up the child. If parents or the emergency contact cannot be reached and the student’s condition necessitates immediate medical attention, the child will be taken to the hospital.
Communicable Diseases
If a child has an illness that is considered contagious, please contact the school nurse. Examples include strep throat, lice, mononucleosis, skin rashes, infections, scarlet fever, chicken pox, impetigo, conjunctivitis (pink eye), foot and mouth disease, and Fifth’s Disease (a form of measles). Children who come to school showing symptoms of any of these diseases will be sent home, and parents will be requested to seek medical attention for their child.
