Required Documents & Fees

To complete enrollment in Fremont School District 79, families are required to submit certain documents and forms and, when applicable, pay student fees. This page explains what is required, when items are due, and where to complete each step. You do not need to gather everything before starting registration. Some items are completed during or after the registration process.

Proof of Residency

Fremont School District 79 is required to verify residency for all students enrolling in the district. Questions regarding residency requirements for the 2026-27 school year should be directed to Lisa Borgford at (847) 566-0169 ext. 4104 or registration@fsd79.org.

What to know:

  • Residency is verified during the registration process
  • Verification is completed electronically using a secure system
  • If verification is unsuccessful, additional documentation may be required
  • Documents must be current and in the name of the parent or legal guardian
  • Registration can not be completed until residence is verified and all applicable paperwork is submitted

School Health Forms

Illinois law requires students to submit health and immunization records to attend school.

Important reminders:

  • Health forms are not required to start registration
  • Forms must be submitted within 30 days of enrollment
  • Requirements may vary by grade level
  • Incomplete health records may affect school attendance

Student Fees 

Student fees help support instructional materials and school programs.

Key details:

  • Fees are posted after June 1 for the 2026-27 school year
  • Fees are viewed and paid through the TeacherEase Parent Portal
  • Families may qualify for fee waivers based on eligibility
