Proof of Residency Requirements
Fremont School District 79 is required to verify residency for all students enrolling. Residency verification is part of the registration process and must be completed before enrollment is finalized.
IMPORTANT: D79 may require any student to prove residency at any time.
Verification of Residency
New Families
During your registration appointment, the registrar will verify your residency using the Residency Verification form. The list of acceptable documents can be found on the form and below (one document must be from Category A & two documents must be from Category B).

Documents must be current and in the name of the parent or legal guardian. Registration can not be completed until residence is verified and all applicable paperwork is submitted. Please note that if you have a child(ren) currently enrolled in District 79, then you are considered a returning family, so you do not need to complete this form for any siblings registering for the first time.
Returning Families
All Fremont School District 79 students must be verified as district residents, as mandated by Illinois Law. We use a third-party service for automatic address verification. If verification fails, a district employee will reach out for additional proof of residency documentation.
Residency Affidavit Forms
In the event a family meets one of the special circumstances below, they may complete and submit the corresponding affidavit form, in lieu of one of the standard Category A required residency documents:
Letter of Residence in Lieu of a Lease - If a family is unable to produce a copy of the lease for their residence, the landlord must complete and sign an affidavit as part of proof of residency.
Letter of Residence with a District Resident - If the person seeking to register the child is living with a district resident without a lease (doubled-up) for purposes other than school attendance, the district resident must complete and sign an affidavit as part of proof of residency.
Affidavit of Enrollment and Residency ISBE 85-51 - If a person other than the child's legal guardian has assumed custody, for purposes other than school attendance, and wishes to enroll the child, he/she must complete and sign an affidavit.

Change of Residence
- Parents/Guardians are required to provide the new address and phone number to the school of attendance.
- A student whose family moves out of the district during the school year may attend school tuition-free for the remainder of the school year. Parents/Guardians are responsible for providing transportation to and from school for the remainder of the school year.
- If the change of residence is due to a military obligation of the person with legal custody of the child, the child may maintain his/her residence for the purpose of registration and enrollment as determined prior to the military obligation. This request must be submitted in writing.
Homelessness/Doubled-Up
If a family is unable to prove residency due to the sharing of housing with other persons (district residents) due to loss of housing, economic hardship, or similar reason ("doubled up"), the student(s) may be eligible for enrollment under the McKinney-Vento Act.
